Description:

PALACOS R+G is
a type of bone cement, similar to PALACOS LV, but it contains gentamicin,
which is an antibiotic used to help prevent infections during orthopedic
surgeries. The R+G
designation refers to the addition of gentamicin to the cement. It is
widely used in joint replacement surgeries, especially in cases where the risk
of infection is higher or in revision surgeries where infection is a
significant concern.

Uses of PALACOS R+G:

Total Joint Arthroplasty (TJA):

Like PALACOS LV, PALACOS R+G is
often used in primary and revision hip and knee arthroplasty (joint
replacement) surgeries. The addition of gentamicin helps reduce the risk of
surgical site infections (SSIs).

Infected Joint Replacement (Revision Surgery):

It is particularly beneficial
in revision surgeries for infected joint prostheses, as the gentamicin helps
reduce the likelihood of re-infection. In such cases, it is used to anchor the
prosthesis and simultaneously provide localized antibiotic treatment.

Fracture Fixation:

Similar to PALACOS LV, it can
be used for stabilizing fractures, especially in cases with poor bone quality
or compromised immune function where infection prevention is crucial.

Vertebroplasty and Kyphoplasty:

The bone cement can be used in
spinal fracture treatments, where the gentamicin aids in preventing infection
when injecting cement into vertebrae.

Benefits of PALACOS
R+G:

Antibiotic-Loaded Cement:

Gentamicin is an antibiotic
that helps prevent bacterial infections at the surgical site by providing a
local antimicrobial effect. This is especially useful in preventing infections
after surgery, particularly in high-risk patients or revision surgeries where
infection is a major concern.

Effective for High-Risk Patients:

Patients who have compromised
immune systems, existing infections, or those undergoing revision surgeries
benefit from the added protection against infection provided by PALACOS R+G.

Low Viscosity:

Like PALACOS LV, PALACOS R+G
has a low viscosity, which makes it easy to handle, fill bone cavities, and
ensure proper placement around the prosthesis. This can lead to better cement
penetration into the bone, enhancing fixation.

Radiopacity:

PALACOS R+G is radiopaque,
meaning it can be clearly seen on X-rays, making it easier for surgeons to
monitor the cement’s placement and check for any complications such as leakage
or insufficient cement distribution.

High Mechanical Strength:

The cement sets to form a
strong bond between the bone and the prosthesis, providing long-term stability
for the implanted device.

Long Track Record of Success:

PALACOS R+G has been used in a
variety of orthopedic surgeries for many years and has a proven track record of
safety and efficacy, with studies showing reduced infection rates compared to
non-antibiotic-loaded cements.

Localized Antibiotic Release:

The gentamicin is slowly
released from the cement into the surrounding tissue, providing a prolonged
antibiotic effect at the surgical site. This helps prevent infection for weeks
or months after the procedure.

Considerations:

Antibiotic Resistance: Overuse
or incorrect use of antibiotics can contribute to the development of
antibiotic-resistant bacteria. However, the localized release of gentamicin
minimizes the risk of systemic resistance.

Exothermic Reaction: As with
all PMMA-based cements, the exothermic setting reaction can generate heat,
which could potentially harm surrounding tissues if not properly controlled.

Allergic Reactions: Though
rare, some patients may have allergies to the antibiotic gentamicin or other
components in the cement.

Summary:

PALACOS R+G is ideal for
patients at high risk of infection, including those undergoing revision
surgeries, as well as those with compromised immune systems. It combines the
mechanical strength and handling ease of traditional bone cements with the
added benefit of antibiotic protection, making it a valuable tool in orthopedic
and spinal surgery.
